A brief word on reports:
Reports can lead our hearts and minds down unhealthy places if we are not embracing the truths of the gospel of Jesus. With the gospel of Jesus, we find our worth and significance in Jesus' righteousness given to us. Without the gospel we are left to find our worth and significance in our accomplishments, performance, and relationships. Without embracing the gospel, reports like the one below can foster pride (when our ministry is doing well) or despair (when our ministry is not doing well). However, when we are embracing the gospel, our ultimate joy is found in the reality that our "names are written in heaven" (see Luke 10:17-20), and so we can allow this report to be simply...a report of the ministry.
With that being said, please give us an honest report on the following questions:
This past semester you planned to help students mature as members of God's family (i.e. mutual care, fellowship, serving on another). a. What were the results of these plans?
b. What were the wins/successes?
c. What were the challenges?
d. What were the failures?
This past semester you planned to help students mature in holiness (i.e. confession and repentance of sin, holding one another accountable, encouraging one another with the Gospel). a. What were the results of these plans?
This past semester you planned to help students mature as ambassadors of Christ (i.e. sharing the Gospel with others, serving and caring for others). a. What were the results of these plans?
How many total college students have connected with your ministry at some point this semester?
How many college students do you consider to be regular participants within your ministry?
How many conversions of college students this semester?
How many baptisms of college students this semester?
How many international students involved in your ministry this semester?
Gospel Saturation How many college students involved in evangelism this semester?
How many college students involved in community ministries/missions this semester?

How many college students involved in one-on-one mentoring, coaching, or formal disciple-making this semester?
